The depopulation and aging of mountain communities, associated with the disappearance of family support networks, isolation and unfavourable socio-economic conditions, are one of the most important priority both in Tarentaise and Valle d’Aosta that require a cross-border approach and the experimentation of innovative models of proximity services that bring a real improvement in social and health welfare.
MisMi aims at answering to the needs of cross-border populations who, due to the characteristics of fragmentation and dispersion typical of the rural and mountain context in which they live, have difficulty accessing the service network. The project would takle the depopulation of mountain areas using technologies to support people. “Family and community nurses” and “community animators” enter into the homes of those who need them together with the technologies of tele-physiotherapy, teleconsultation and tele-medicine, to support it in maintaining its state of health as much as possible and well-being through safe assistance even at a distance.
The project promotes the empowerment of people and communities in the area with social and health proximity services and the improvement of access to services as well as their quality, using new technologies. Thanks to the multi-professional teams and to the involvement of caregivers and even informal solidarity networks existing in the area, the integrated assistance and treatment paths activated with MisMi to take care of the chronicity together with an initiative of initiative medicine, can contribute to reduce isolation and social and health inequalities, as well as to improve the quality of life of mountain populations.
